The two documents are required when you come in for your visa medical examination.
- Valid Passport
If not available, the following identification documents may be presented:
- Certificate of Identity
- Refugee Travel Document
- Letter/Medical instruction for Medical (if available)
- Minors (18 years old and below) must be accompanied by their parent or by a legal guardian who has thorough knowledge of the child’s medical history.
Whenever applicable, please bring the following:
- Old chest x-ray films not less than 3 months prior to the medical examination
- Medical certificate regarding a previously-treated or currently managed clinically significant illness. The medical certificate should contain the following information:

- name of medication (brand and/or generic name)
- dosage
- duration and dates of treatment
- surgical procedure done
- complete final diagnosis
- outcome of treatment or treatment plans and prognosis, if available
- Physicians’ affixed signature above printed name
- Physicians’ clinic address, contact numbers and email address.

Specialty consultations/reports available within the clinic are charged accordingly. Any other additional tests required by the specialists shall be individually charged.
